Is this a first time brewing this recipe?
Have you used the brewing calcuators to verify your expected OG?
It might be any number of things, more likely the crush or something like that.
Where did you get the grain? Was it something you purchased in bulk a while ago or at a LHBS or online? if either of the latter I would surprised if it was expired (past it's prime) any of those places should go through enough grain to make that unlikely at best.
I have a recipe for a pale ale that uses 12 lbs grain - 11.5 pale and a half pound of light crystal malt, and I end up with about 1.050 OG on that one. I'm wondering where the calculations mess up - your end or mine.
 
It was the crush. I have repeated the same process with multiple recipes. Local store has a hand crank mill, friend crushed the first batch. He didn't noTice one side was loose. When I crushed mine, I looked and tighten it up.

Unfortunately the low gravity beer was his. 9lbs of grain, with my normal efficiency of about 75 should have everything put it at 1.050 or so. Finished at 1.032. I had some lme so it was added late. 1.46 now...it will be drinkable...
