ITC-1000 & Freezing Keezer

Looking for something obvious that I may be missing... I'll try to keep it short for everyone.

I converted a side-by-side to a keezer/fermenter (left side keezer, right side 2 fermenters each individually temp controlled using ITC-1000 to control the temperatures). I bypassed the side/side thermostat and use the keezer side ITC to control the on/off of the compressor. The 2 fermenter side ITCs turn on a fan for cool and heating element to heat.

I have had the keezer side set at 38 deg F with the the remaining settings (TS, PT, CA) all set at a value of 3, and it has been running solid and cold since Aug 2018.

Today, I was down in the basement and noticed the temp was -4.7 and all lines in the keezer (left side) were frozen. Settings were still the same. It got down to -15.6F before I pulled the plug. Crazy thing is that this is the 2nd time this scenario has happened. After the first time Aug 2018, I just ordered a new ITC-1000 for the left side.

Is it possible that 2 controllers could go bad? Was I just lucky to have it running stable for 6 months?
