Well I just had a doozy today. Went down to the basement, notice a lake of beer on the floor. Apparently the picnic tap on the keg that held my IPA that I kegged last Saturday was faulty resulting in all five gallons of beer to flow out into my fridge and all of the floor. I pulled a pint last night to see how she was tasting (darn yummy I must say). I noticed some beer streaming out when hooked the tap up to close the fridge so I readjusted it, and everything appeared to be fine, Oh how naive I was. Oh well, at least clean up was easy, squeegee the beer into the floor drain, rinse with water, let it dry. Could have been worse if my kegerator was a main living area or something. Lots of time, money and beer wasted, though. :(
 
Fortunately, I haven't had too many incidents while brewing, since I read these threads and the ones about injuries, and am pretty fastidious about sanitation (some other things not so much, but that's the big one)
There was the time I was culturing dregs from a bottle of soured beer, had it a gallon jug. I was walking over to the slop sink to decant the spent wort to pitch the rest, and dropped it. No injuries, (as I said, I've read to many of these so I always wear jeans and boots when brewing and dealing with beer in process) but it sure smelled great as I mopped it up and swept the broken glass.
Another time I bottled into a couple growlers along with regular bottles (I've done this on and off all along, and have never had issues with losing carbonation over time.)
THis one was a hefeweizen, and I later found out, the growler used was very thin glass. I had noticed that one was lighter than the other I used that day but didn't think anything about it. Anyways, middle of the night I heard a pop, couldn't figure out what it was.
The next morning, I'm walking into the bathroom, past the cabinet the beer was stored in, and realized that I was sticking to the floor. Durn thing let go in a perfect circumference and inch and a half from the bottom. Did not enjoy cleaning that up before going to work...
