Sweet off flavors

Hey everyone, i am having a problem with some of my beer. I am getting a cough syrup like off flavor both aroma and taste. sometimes it is worse than others and i cannot seem to find a consistant reason. i was wondering if anyone else has had this problem and i woudl definately be pen to all suggestions. mostly i do 10 gallon batches, and culture my own yeast, but i did try a five gallon batch and a packet of yeast and wound up with the same problem. in some beers they are drinkable, but others have not been.

Sum it up, "probably" chlorine in the water, residual bleech from cleaning your gear. Could also be caused by pitching yeast when wort is too hot, can also be caused by wild yeast infecting it.
