How do I determine how much starter I need to use?

I would like to know how much starter to pitch depending on gallons being brewed. Do I use 3 times as much starter on a 15 gallon batch than a 5 gallon batch? Is it true I do not want to use starters on Hefe's due to the esters? Any other styles were starters are not desired? Do microbreweries typically use starters?

The longer answer is It depends largely on gravity how much yeast you should pitch for a healthy fermentation.

Thank you very much. I assume Beersmith 2 has this also, but can not find how to make it do the math for me in the yeast starter tab.
