High ABV...What am I doing wrong?

The last three brewdays have ended up with a higher OG, eventual FG/ABV than my target. It is usually approximately 1 - 1.5% higher than expected. The beer doesn't taste bad, but am tired of the rocket fuel. I have tried following recipes to the word, but regardless it still happens. I do all grain brewing and have had looking nearly hitting OG and FG spot on, but would like to do it on a consistent basis. Any suggestions?
 
Scale your recipes down according to your efficiency. It's a good thing, because using less grain means saving money!
 
Also might want to check recipe sizes, in case they're for 6 gallons instead of 5 gallons. For example, if you use recipes out of Brewing Classic Styles, those are all for 6 gallon batches.
