Beer Greeting Cards - Survey for Class

I am working on my MBA and for class we had to come up with a mock product and go through the motions of doing market research. Of course I decided to do something beer related.

If you could please take my team's survey on greeting cards that attach to beer/wine/liquor bottles, we would much appreciate it. It is all completely anonymous and should only take a few minutes.

My only feedback on the card frequency was that my choices are:
Never
Less than Once a Month
Once a Month
2-3 Times a Month
Once a Week
2-3 Times a Week
Are there really people that purchase greeting cards this often? I mean, I probably do cards a few times per year. A few times per week? Holy cats
