Low OG

So I'm about 4 batches into my all grain brewing career and i continually have a low OG reading, roughly 1035-1038. My mash is normally about 70 mins with a temp range of 152-157. Any ideas why I'm getting such a low OG?
 
it's being milled at the supply store. i just run it through the machine. it's crushed but it doesn't look like it's being torn up too much
 
Might want to try the following:

Tighter mill gap, if the store will let you adjust it.
Calibrate your thermometer and make sure that you are mashing at the temps you think you are mashing at.
Use a pH buffer like 5.2 or find another way to control your mash pH
Make sure you are mashing somewhere in between 1qt/lb and 2 qt/lb
Check your sparge calculations and make sure you are not leaving any sugar behind in the mash tun.
