Cant hit Target Original Gravity

It's put my Ommegang Hennepin clone into the carboy. Took measurement of original gravity and came in at 1.058 vs. target of 1.072.

Any help explaining possible causes would be appreciated, thanks,
 
Is this an all grain, extract with grain or partial mash recipe? Is this a top off batch, where you added water to the fermenter?

We can't really help you without details.
 
The recipe called for liquid (8 lbs) and dry malt (1 lb) extract. I steeped some grains at 155, per the recipe's instructions, for 30 minutes. Boiled for 60 minutes.

The wort was at 73 when I checked the gravity.

Can I hijack for a quick second?

I brewed my second PM batch on Friday. My OG was .013 too low. Sort of a big amount, at least in my eyes.

I had trouble with my mash. I couldn't keep the temperature consistent. I used a cooler for the first time, and it was just all too cold. Could this be the reason why my OG was low?
