What happens if you do not get a full conversion?
Does it cause any off flavors?
What is the best method to make sure you have a full conversion?

No one is able to get all the sugars out, that's why recipes adjust based on expected efficiency. Most homebrew setups get about 75% efficiency. I have a very simple AG setup and get that.
