My first Cider, need input please.

I thought I'd try a cider, I'd like to try some 1gal batches for practice. I have 4 one gal glass wine jugs, I will pick up stoppers and airlocks next time I'm in town.
I also have 6 good used Grolsch beer bottles and a bunch of reg green beer bottles.

What I'm looking for is a semi sweet bottle carbed cider.
Here's what I was thinking about trying, let me know what you think.

1 gal Apple Juice, cheep stuff w/o preservatives.
1/4 can frozen White Grape Juice Concentrate
1/2 cup brown sugar
1/2 cup white cane sugar

I have some safbrew T-58 yeast in my fridge, or would a another yeast be better?

One week in the Primary, two weeks in the secondary (?), add 1 pint (?) additional apple juice and 1 oz (?) priming sugar and bottle.
After a week or two place the bottles in a pot of 160F water for ten minutes to stop carbonation (bottle bombs)

Any thoughts and tips would be appreciated! :D
