When did you pitch the yeast? Did you see signs of fermentation? With 12 # grain(assuming it was all fermentable) you should have had an OG in the ballpark of 1.060 - 1.065. It's possible 1.025 is your FG..but you didn't get very good attenuation. Assuming you pitched the yeast on the same day...

You're fine with extract. It's been boiled enough to remove dms.

Just add more hops for the 60 minute boil. Unless you are using pilsner malt, it's HIGHLY recommended that you do a 90 minute boil to rid the wort of dms.

I loved the yeast book! There was a lot of information that went over my head..but i learned so much from it, and I also use it as a reference book too. I need to get the rest of the series, I definitely want to read the water book next.

Did you reach your estimated FG? How much sugar did you use, and how many gallons of beer did you bottle? What temp. was it stored at after bottling? There are many, many reasons for bottle bombs.
