Southern Tier Choklat recipe?

Anyone have the recipe for choklat? such a good beer....

This is the label with the grains on it, but with no proportions.

I know for sure that they use pure chocolate extract at bottling and not cocoa powder. Not a lot of help I know, but that is how they get the extreme "hershey bar in a glass taste".

You can see the brewery from my parents house and I'm there all the time when I'm in town visiting them.
