Problems with uneven carbonation

I've searched on this but have not gotten enough evidence about what
could be going wrong.
After fermentation was complete, I waited 3 weeks for my beers to carbonate and I have moved them into the fridge. I boiled 5 ounces of dextrose in water and added it into the bottom of my bottling bucket. I then made sure the tube of the siphon when racking was under the bottom of the water and filled up the bottling bucket.
I now after 3 weeks have some beers that are perfectly carbed, some a bit flat, and some gushers. I'm wondering what could cause this?
Will time in the refrigerator help? Should I try stirring next time.
I'm a bit irritated, because I just used this procedure on another batch and I hope they aren't as uneven as this one.
 
I keg most of my beer, but I use your same basic procedure you do when I do bottle. I do usually gently stir it. I just stir enough to get it moving in a circular motion around the bucket being very careful not to splash. Works great for me.
 
Could be a problem with your sanitizing process. Walk us through how you sanitized the bottles before use. Are the bottles used?
 
I had a bucket full of star san and dunked each bottle in the bucket, made sure the solution got in the bottle, poured it out, and placed on my dishwasher rack to dry.

They were recycled bottles, but made sure to rinse them before sanitation.
 
I had a problem when I started with not capping the bottles tight enough. Some did fine others took longer to carbonate. After my second batch I started crimping them all tight and didn't have a problem.

The gushers.... Did you refrigerate them for at least 24 hours? Opening warm might cause that. Also was there a large amount of settlement in the bottom of any of the gusher?
 
Sounds like they're not quite done carbing yet. Remember three weeks is only a guideline. Give them a bit more time.

It's weird, the blonder I bottled over a week ago with table sugar, I just popped it and it's perfectly carbed!
I know table sugar is more effective so you use less than dextrose, but could the dextrose behave differently or something?
Of course they are different brews so that's a factor too.

1) Star San is a wet contact sanitizer so it should be wet when used, not left out to dry.
It needs at least 30 sec to work.

2) I would soak in oxyclean overnighy and/or use a bottle brush to really clean the bottles, not just a rinse.

It varies from beer to beer. Some take a week. Some take a month. Depends.
