loses beer head

I made a ''noble trappist ale'' using trappist activator yeast, 3 weeks in the fermenter and two weeks in the bottle. When I pour it in the glass, I get a nice head, but about 10 seconds later, the head disappears, it's carbonated o.k. The the way the head just goes away is very odd. Does it need to be in the bottle for a couple more weeks?

I have had a couple brews do this because I did not let them condition in the bottle long enough before putting them in the fridge. They seem to have good carbonation, but they were not quite ready.
 
dirty glass could be it, I will try cleaning the glass better, if that dosen't do it, I will let sit in the bottle for another week, thanks for the feed back.
