Looking for the cause of a very low carbonation

I brewed as per my usual procedure with a couple changes that don't explain a quasi-flat beer in my opinion.

Maybe something rings a bell:

- I got a lower than expected OG for 3 reasons: 1. I didn't sparge as much as usually, 2. The boil was pretty quiet and, hence, caused less evaporation 3. A small leak in one of the tubes of my immersion chiller added some water to the batch, but not much
- I forgot to filter the hops when transferring to the fermenter like I usually do, so they spent 2 weeks there, as well as the ones I added for dry hopping
- I reduced my primming sugar amount by approx. 12%

The beer is litterally flat and I don't think any of the above factors is significant enough to explain this.

Any idea?

What was your OG/FG, yeast used, batch volume, amount/type of priming sugar, temperature at which bottles were conditioned, length of conditioning time (before sampling)?
 
Storage is one factor. It is no longer summer and with colder weather perhaps your storage area is colder too. That keeps the yeast slow/dormant and you get no carbonation.
 
different brand of cheap caps? old capper not crimping good? (turn a couple upside down for a while to find that out)

When i used to bottle carbonate, sometimes i'd keep them for a few days a couple feet away from one of my radiators in the back bedroom. Carbed much quicker than just keeping them somewhere at closer to ambient house temps
 
Just as a follow-up:
I left this beer for another 2 weeks in a warm place (77F/25°C), because I suspected the bottle carbonation to be somewhat dormant and it worked for the most part.
The beer is still relatively mediocre, but it's carbonated and will probably be quite decent in 1-2 weeks.
