lager bottling question

first post so bare with me. I usually find what im looking for with a search but couldn't seem to find exactly what i was looking for. I made a baltic porter with wyeast 2124 Bohemian lager yeast and did a sudo lager fermentaion. started with a week in primary in the high 50's then transferd to a keg and now is in my keg fridge for 6 weeks at fridge temps 42ish now its time to bottle.

question 1 do i add any yeast at bottling?
question 2 what temperature do you bottle condition at?
question 3 how much priming sugar (5 gallon batch) same as for ales?

If it's in a keg right now why not force carb it up and then bottle straight from the keg?
 
Thanks I would just carb it in the keg but I don't have any way to bottle effectively its on my list
