How's my head space look? Appropriate?

Also, I haven't been able to get a pure oxygen injection solution figured out so I'm just swirling for 5 minutes before pitching. Do others do another shake up @ 12 hours or no?

That's too full for a primary but looks good if transferring to a secondary for extended aging/adding fruit/vanilla/etc.

Also if you don't follow proper fermentation temps for the yeast strain you'll be blowing out easily. As far as getting oxygen in the carboy, good quality shaking (not swirling), for about 1 minute is all that's needed. Check Wyeast's site for more details. No additional shaking is needed if pitching healthy yeast at the correct pitch rate.
 
Definitely get a blowoff tube on that bad boy or you are will likely have a mess.

If it's any normal strength beer, shaking vigorously will work just fine. You really want to shake it though, not just a gentle swirl.
 
Agreed, if that is primary you have either collected too much wort if a kit, or a lot if that is what you were looking to get. That space will quickly fill and likely blow off if primary.
If so get a blow off tube on it ASAP.

If secondary it looks good.

Since fermentation creates co2 which is heavier than oxygen, too much headspace in primary is not really much of a concern.

After you have pitched the yeast you usually do not want to aerate any more. With most brews that would be bad, risking oxidation.
