A proper Black and Tan is layered. Personally, I think it's more for show than anything else, on an equal footing with flamed drinks.

Also, I've been led to understand that it's not advisable to order one in Ireland.

Only order it in Ireland if you want them to make you a British Redcoat (Red Bull, Ever-clear and a tablespoon of ketchup), or a Zimmerman Telegram (Two shots of Jaegermeister on crushed ice, served by throwing it in your face and telling you to leave the bar, please).
 
I see three trends here, those that expect to go to a local bar and order a mixed beer, those mixing beer at home, and those at the local micro. I can see the reluctance of a bar from layering a black and tan, home brewers it's whatever you feel. Local micro, you know the barkeep,the head brewer may be there, they know your name, it's the back and forth, and hey lets blend. The head brewer doesn't care, they are looking for recipes and maybe come up with a new style, it's all good.
