Beer tasting score sheet

I am having a Beer and Wine tasting party at my place and am wondering if anyone has a good score sheet to evaluate beers?

I have some beer guys coming and I am not talking about the bud light kind. I want to see if we can guess the stats of the beer essentially such SRM, IBU, alcohol, maybe even OG and FG along with the other main stuff.

Has anyone done with or know of a good resource?
