Here is the recipe I brewed this past Saturday. This was a 4.5 gallon BIAB.
4.75 lb German Pils
2.75 lb Munich II
4 oz Carafa II
4 oz Black Patent
4 oz Chocolate Malt
4 oz C40
0.5 oz Perle FWH
0.5 oz Tettnanger 10 min
0.5 oz Tettnanger 5 min
0.5 oz Perle 0 min
Dry-pitched US-05
I hit an OG of 1.056 and ended up with just over 4.5 gallons, meaning my efficiency was close to 85%, which is about 20% better than my previous two BIAB batches. It's bubbling away like crazy right now, so we'll see how it turns out in a few weeks
