Inconsistent bottles from Deception Cream Stout

I made Deception Cream Stout from the recipe lists with a few modifications. It's been in the bottle for about 5 weeks, started tasting them about 3 weeks out. All of the bottles have been different. Different carbonation, head, mouthfeel, even different flavor. Some have been more or less creamy than others. Just about everything has been different. Any idea what went wrong?

Here is my recipe.

3.3 lbs Breiss Amber LME
~1.5 lbs Breiss Amber DME
1 lbs Bavarian Wheat DME
1 lbs Chocolate Malt
.75 lbs 60L Crystal Malt
.5 lbs Roasted Barley
.75 oz American Magnum (60 min)
1 Whirlfloc (10 min)
½ lbs lactose (10 min)
½ tsp Wyeast yeast nutrient (5 min)
1 pack Safale S-04

Fermentation room temp: 63-64.

OG: 1.061
FG: 1.027

In primary for 3.5 weeks. I figured the high FG was from some poorly measured lactose.
 
What method did you use to bottle condition? I have a feeling that you may not have distrubeted the priming sugar evenly through out the beer. Differnt co2 levels can change all of the above characteristics you mentioned
