Missing gravity

Inconsistently missing my gravity by 10 points. I've checked all the obvious things. Calibrated my hydrometer, checked temps , measured accurately. I did a double IPA today and my OG was 1.066 and it was supposed to be 1.078.
I use a barley crusher and have a fairly tight crush. Any advice you can give you be appreciated
 
volumes on sparging? maybe you're not actually getting all of the converted sugars off of the grains, even if you're converting them appropriately.
 
My boil is vigorous. I guess I need to go to beer smith and put in my actual numbers so I can get my efficiency correct.
