when to get your OG

Is the OG based on the 2.5g of Wort, or the Wort mixed with top off water before pitching? So e.g. the recipe says 1.084 OG, is that the Wort mixed with the 2.5 to 3 g of water or before? A lot of posters including myself have been coming up with a lower than targeted OG for some reason, but my beer seems strong as it should be.
 
Measuring OG on an extract batch topped off with water is a crap shoot. Every thing has to be mixed perfect or you get a misleading number. If you followed directions, your OG will be spot on
 
Mine have been close, but not spot on they have been on the low side, if the OG is supposed to 1.050 - 1.055, I'm getting 1.044 ish (just an example), I don't alter the extract recipes in any way and I'm using 2 hydrometers to verify readings mixing well, compensating for temp etc. etc
