That seems almost like there would have to be a bad seal on a fermentation vessel.
I mean... Gross, but that has to go beyond not sanitizing?
 
Yes that story seems suspect, but that would come from allowing lots and lots of flies to have access to finished beer I’d guess.

Would they even be able to survive if it's so CO2 dense? Something is afoot.

I did some digging. I can only find stories of this from people with very long-term (usually secondary) fermentation and a poorly sealed vessel.
It seems gnats or fruit flies are good at getting in to small places with fermenting liquid... So keep your seals strong!
