First ever sour. BIAB.

Brewed the recipe today nearly exactly from the following link.

We didn't do a mash out and I adjusted the recipe to be 6 gallons. As we frequently end up with 6.

Our OG came out to be 1.037, and we ended up with 6.5 gallons. We average 80% efficiency and I am perplexed as to what happened to cause our gravity to be so low tonight. I have ended up with an extra gallon before and still hit gravity.

Was it the 30 min mash?
Should we have done the 60 we normally do?
Was it the extra half gallon I didn't plan for?
Should I add DME as per this article? http://beersmith.com/blog/2015/02/24/correcting-low-original-gravity-in-all-grain-brewing/ It would be 2.36# of DME.
