Reliable Refractometer (post-fermentation) adjustment

Can anyone provide a Reliable Refractometer (post-fermentation) adjustment calculator or formula?


FYI.
I searched the forums but none of the results were helpful at all.

Sean Terrill's calculator has worked well for me with results that correlate with my FG measurements by hydrometer over greater than 30 times. I have always used the standard wort correction factor setting of 1.040.
