Recipes and 5 Gallon Volume Question

If I brew a recipe whether a kit or found online and it says its calculated for a 5 gallon batch, does that mean that 5 gallons is measured as:

-Total volume in the fermenter including trub?

or

-Volume in the fermenter excluding trub, dry hop absorption, etc.?

I don't want to fall short on volume and have an excessively bitter beer.
