Okay...

That was a Wednesday brew.

I was brewing a Mild. Using Beersmith, I came up with a bill grain that sortof made sense, considering the style, and that gave me an target OG of 1.036. Using a Brewhouse Efficiency of 72%, with my regular settings, I ended up with a pre-boil gravity of 1.022 (obviously hydrometer-adjusted). The "target" was 1.021, so I was basically dead on.

I mostly did everything I always do, sparged like I always do, and boiled like I always do -- 60 minutes.

But I ended up with an OG of 1.031. In most cases, being 5 points under target isn't really a big deal. But we're talking about a Dark Mild here, and that's some big undershoot.

Soo.... my guess I that screwed up some Equip Settings in Beersmith. This is not the first time that my pre-boil is okay, but my OG is quite a bit below where it should be.

The beer prior to that one? Pre-boil 1.038, while "target" was 1.035. Still ended up with OG below target (1.058, while target was 1.060, thus, that was really not a big deal).
The one before? Pre-boil was 1.033, while "target" was 1.028. And my OG was a whole 6 points below target (1.048 instead of 1.054).

(And prior to that, I wasn't keeping track of pre-boil gravity).

Sorry for the somewhat aimless post, but I'm not quite sure of what's going on. My mashing eff. seems pretty good, but my efficiency as a whole is pretty much down the drain. Anybody experiences the same "discrepancy"?
 
How sure are you of you volume measurements both pre- and post-boil?

By volume, you mean water or density?

Taking the sample from the fermenter, instead of from the kettle might have tweaked things just a bit. Plus, I ended up with too much boiled wort +- 200 ml (either because I was a lil' too cautious and overrated my boiloff rate, or because today's wort was incredibly clean for my standards - that's what happens when relying on "visuals" on the kettle for quantities).

The worst thing is -- the sample from that Mild was incredibly tasty. It had quite a bit of body. It also had a 1.031 gravity. I also mashed quite hot (on purpose).
