OK, I think I got it right, but just wanna make sure.

When doing a partial boil, you are supposed to take the OG of the full diluted 5 gal yes? (not just the couple of gallons of wort in the kettle)

Yes, the OG is always of the full wort in the fermenter. You have to stir it really well if you're topping off with water, since it seems like the wort with the extract in it wants to sink to the bottom. Stir very well, to thoroughly mix the wort before taking a sample.
