Over carbed batch

I brewed a batch of "mild" back in June. One of the bottles broke on the shelf in the basement. There's a lot of pressure upon opening a bottle; it foams out until over a third is gone. Pouring a glass is pouring a glass of foam. Would over priming cause this. this is about my 15th batch and the first time this has happened. OG was 1.040, FG 1.013. 1 week in primary, 3 weeks in the secondary. Yeast - Safale S-04.
 
With the extended secondary it seems like it wasn't bottled to fast. So i would say that to much priming sugar could be a cause. I'v heard there are infections that cause over carbing as well however.
