I took the sample after adding ice to the wort but before shaking the bucket. I noticed the sample was much lighter in color than the wort but didn't think much of it until the reading later came in at 1.01. I am thinking it may have primarily consisted of water that melted off the ice.

Is the OG important enough to use a wine thief and open up the primary for another sample before it starts fermenting (it has only been 1 hr)?

Or do I assume it was close to the 1.04 that the extract recipe stated?

With an extract recipe, so long as you use all the ingredients and the right amour of water, you are pretty much guaranteed to hit your gravity numbers. No worries.
