It's a year old. Don't know what if anything should be replaced. It's for a pumpkin stout. Everything is in a sealed bag but the grains are covered in something black and don't know if they should be...

Year old - I'd pitch it. If the yeast was not refrigerated and the hops were not frozen those are DEFINITELY worthless. The DME would probably be okay, but I doubt the LME would be.
 
Get some fresh crushed steeping grains and fresh hops and brew it. The dry yeast is probably fine. Check its expiration date. Dry yeast is durable.
