Without tasting I can only offer direction. However you do have a lot of info here to work with.
You can assume with it being American ale yeast that cal ale yeast by whitelabs or s05 safale would be a good selection for yeast.
Knowing the apparent attenuation of the yeast, plus the efficiency of your system, along with the starting gravity, you can calculate the total amount of grain.
It looks like there are only 2 grains. As a general rule of thumb you would not use any more than 20% crystal malt. Probably less. Pour the beer in a glass and use the BJCP color chart to get SRM, and that can help with total % of crystal.
As for the hops, if the bottle or website lists total IBUs that would help. If not you will have to rely on your tastebuds. How bitter is it compared to a know IBU beer? Anything over 60-70 IBUs is really hard to distinguish. That is pretty much the taste bud threshold.
Next use your nose. I would assume that the EKGs would be used for aroma, so sub 7 minute addition, including flame out. Centennial and Newport are probably flavor additions, so 30-15 minute additions. As far as the bittering, it is not as critical because you will (mostly) lose the flavor contributions. For bittering you just need to calculate IBUs based off the hops AA.
Like I said, without tasting it I can only offer guidance, but arms with a little info, and some tastings, you would be surprised at what you can accomplish.
Edit* just looked at the website, the SRM looks to be around 9-12, so probably around 5-10% crystal malt.
IBUs at 15, so not very much hop additions at all.
