I could be wrong (in fact there's a good chance) but I believe that is a Taprite secondary regulator, and if true your CO2 cylinder pressure is way above the regulator's input rating of 60 psi max.

Is there any chance that was cobbled together and the wrong body was used?

[edit] Looked around and found Taprite primary regs with gray knobs, so scratch that (plus the extant high pressure gauge should have tipped me off ;))

Cheers!

I just put a new gauge on it because other one was messed up and I had it set at 10 psi and it leaked took it up to 30 and you can start hearing co2 leaking from the holes

Took the safety release out and theres 3 o rings and the spring, I'm guessing its leaking around the big o ring which mean the spring might be weak
