A while back I stopped milling my grain at my LHBS and bought a crankandstein mill. My mashing efficiency dropped from 75 to 65%. Mine is a set gap mill, but it can be taken apart and adjusted then put back together. My question for everyone is how fine the grist should be. I've tried one batch with course and one with fine and both ended up the same. Any links that describe milling grain would be helpful.

also I bought all of my grain in bulk back in Feb. so I'm wondering if it's just getting old.
 
You should crush the grain until you are scared.
Depending on your constitution, maybe you should crush it some more. If you are having no problems lautering then you aren't crushing enough.
