When I had the same problem, it was because of the size tubing like Arcane said.
 
I had a similar problem and Revvy recommended a small hose clamp which is available at most LHBS's. Mine didn't have the particular clamp; however I was able to solve the problem by pushing my hose as far as I could onto the racking cane, almost all the way up to the bend. Just don't go up into the bend, as it can force the hose open a bit and allow air in.
