I came close, as I have three in the closet right now done within the same week. I had to smell the air lock and study the over flow from it to confirm which was which.
 
A postit note usually does it for me, most times I write my shopping lists on postits and then when I get back from the shop I just stick the list on the carboy. I have confused a blonde and a cream ale, and added rose hips to the wrong beer. Oh well.
 
After a group buy I was putting grain into buckets. I poured pilsner into one bucket and 2 row into another without labeling them. I had to do a taste test to figure it out. To this day I still think there's a chance I got it wrong.
 
That my bottling sugar is in the fridge cooling after a quick boil and go ahead rack and bottle my beer... Only to find the small pot in the fridge the next morning and have to run to the LHBS to get some bottling tablets.

Been there done that. Worst feeling finding that sugar after everything is bottled, boxed and cleaned up.
