For some reason I can't seem to get my brew to pour correctly. My beer is kegged and I've had it conditioning for a month. One big problem is that I left it at 30 pounds of pressure the entire time instead of for the first day or two. On my first attempt to pour it came out looking like soft serve ice cream, so I let all the pressure out for a few days. When I gave it another shot, adding 10lb of pressure, It wouldn't come out at all, so I increased to 15, nothing. Then I went up to 20lb and it was back to soft serve ice cream again.

I've changed the hose and that had no effect. Any suggestions?

Unless you have a way to measure the actual carbonation level you might want to de-carb your beer and then start over, because at this point its hard to say what the carbonation level is.
