What would happen to the wort in extract brewing if boiling temps were not reached for the full 60 minutes?

Hops utilization may be a little off but you still made beer. Did you reach a boil at all or was it just not for 60 minutes?

I reached boiling temps but wound up starting the 60 minutes before I reached them. Didn't realize It starts once boiling starts. How bad did I screw up?

You will be fine. You will lose some hop utilization, but not much. For reference, the difference between a 50 minute boil and a 60 minute boil is about 5%. So if the IBUs were projected to be 40, you would have gotten about 38. Way too small a difference to be detectable.
