Beer Entry Recipe Form

I'm entering a couple beers in my first competition next month and was looking around to try and find a BJCP form that lets you type in it.. Since its a pretty standard form with a bunch of small boxes I wanted to make one so all of you can use it as well.. i wouldn't want anything to go wrong with the judging because they couldn't read my handwriting..
