The SOP is to take a hydrometer reading to see where it's at. It's possible it already fermented (maybe day 4), but did so relatively quietly. You can always add more yeast if it hasn't fermented yet, or put the fermenter in a warmer place, or jostle the existing yeast a bit to get them back into suspension.
 
Making beer is one of the weirdest, sometimes most unpredictable things i've ever done. I've had a 1.040 simple blonde BLOW the top of the airlock off and paint my fridge with krausen, and in the same way had a 1.100ish imperial stout barely create a krausen. You really can't judge whats up with out taking a gravity sample.
