Yes. Those are crap. A solid rubber one or the orange cap does the trick for me. I use that one for my PET carboy without issue.

Yes, mine likes to work its way out too. I gave up on mine and went with something else.
 
I gave up on those things and just use carboy caps now. IF I'm feeling worried about the seal I secure it with a worm clamp.

One of my universal bungs likes to work its way up. Don't know why it is just one, but a piece of copper wire across it keeps it in place.

That's a good idea, don't know why that never occurred to me... Or use a wire hanger.
 
ive found once they stay in place for a whole fermentation they taper in from being in the neck a bit. From then on, they work perfectly. Once or twice ive had to use duct tape to keep a new one in place for 2-3 hours. Once the starsan on it dries, it works fine and stays put
 
I had that problem when I was using starsan on my bung cover. Now I have a little bit of vodka that I'll put my bung cover in before I put it on the carboy. The vodka helps keep things from slipping around too much.

This is my idiocy/lack of using those, but what are the two opening for? I assume one is for the bubbler, other for...?

A bubbler can go in the short wide one during fermentation. Afterwards the racking cane goes in there while the longer skinnier one is for venting and to kick start the siphon process by blowing in it.

Does the orange one fit PET carboys? I just got a PET carboy and need to get a cover still.

Sure does.
