Swapped Co2 mid-keg - psi level help

So my CO2 ran out mid-keg... I had it running around 10-12psi. I disconnected it, filled it up, reconnected it, and it instantly shot up to ~25psi. I tried to turn down the regulator and bleed off some pressure, but it wouldn't come down.. what am I doing wrong? :confused:
 
Do you normally keep your tank in the fridge and replaced it with a hot tank? The pressure may run high until the tank cools.
 
Where you adjusting up the pressure as the tank was running out? Thats my last guess.

OK maybe I have one more. If you disconnect the gas at the keg, turn the reg all the way down, bleed the pressure off, can you reset the reg?
