Question about boiling hops.

So I will be brewing my second batch this weekend and it will be the first time steeping/boiling hops. (First batch was a pre-hopped Mr.Beer extract kit.) I know that steeping specialty grains require a mesh bag. My recipe requires 15 minute hops, 5 minute hops, hops at flameout, and a dry hop 7 days into primary.

My question is, which of these hop additions should be in a bag and which should just be thrown in. And when should they be taken out?
 
Okay, so it is perfectly fine just throwing them in the wort at each stage of hop addition?
 
Yes, that's what I do too. Just toss them in, it's fine. Some people strain the hops from the boil into the fermenter, but I just toss everything in the fermenter as well. No worries
