Lets form a small checklist of things that will increase efficiency. Without debating so much the merits of each, just simple things one might do.

Long slow fly sparge
Double crush
mash Ph set to 5.2
Thinner grist ratio
Temp rest @ 115F 30 mins to break down large sugars.
Study grain specs like % moisture, starch content
Longer mash time.
stir mash well before sparge.
proper grainbed depth with good design drain that rinses all grains
collect extra wort and boil off longer.

What else?
 
condition your malt and then crush it. (moisten it with a measured amount of water before crush)

or just get a decent crush the first time. That is where I see the biggest effect on efficiency. And learn your system. Every system is different and will work better with different conditions.
