Missing OG

Brewed a batch tonight that Beersmith says should've had an OG of 1.048. After adjusting for temperature, I'm at 1.06. What am I doing wrong? I have been consistently missing OGs for the past few batches.
 
could be several things.
1. Your beersmith "efficiency" is set to low (eg 75% and you are gettin g>80%).
2. You are not adjusting your hydrometer for actual temperature
3. You are adding more sugar than you thought (scale off, malt extract off...)
4. You are not really not really getting 5 gallons (if that is what beersmith is calculating. ) If you have less then it will be a higher fg
5. You haven't shaking well enough prior to taking your sample and you are getting more concentrated wort without your top off water.

There should be an adjustment option in the settings area, if you're asking a mechanics question. But the only way to figure what to set the efficiency to is to measure your pre-boil SG (chilled to 65F if using a hydrometer) as well as your post-boil SG and then do the math to get the percentage of extract use.
