After you drain off the first runnings, you'll still need to sparge so that makes up the difference. Most calculations will give you somewhere around 1.5x-2x as much sparge water as your initial mash, so you just draw off what you need to make your boil volume.
 
sooooo... what's the question? what are you looking to solve?

He was venturing into all-grain and asked how to get more than 5 gallons of wort in the boil kettle. He was doing a single infusion mash + mash-out calculation, and didn't look like he was accounting for sparge water.
