Missed pre-boil volume Decoction mash

I use beersmith to set up my first decoction mash. Everything went pretty well except I missed my pre-boil volume by 3 gal. I normally have about 13 gal but ended up with only 10. I used water to top off and hit 75% efficiency. That was ok, but I wonder what went wrong. Also, I would imagine I could hit a higher efficiency if I had the proper amounts of sparge water.

Basically if I use beersmith for a decoction mash I want to be able to trust it.
