It depends on if I hit my preboil gravity.
If I did, then I change nothing, and make extra beer. :ban:
If my preboil gravity is too low, I extend the boil off rate to reach my anticipated OG. I have slight variations in boil off rate, so I have a range I'm shooting for. So if its just a...
ACK!
Never had any problems with Maris Otter on my hand crank malt mill.
My LHBS has told me their mill always gets messed up when customers adjust it (without doing it properly), so I wonder if that could caused your problems?
When I am feeling lazy (and hell that's most of the time :D...
I'm still hand cranking, so its easier just to feed the grains through twice. If I ever get off my lazy butt and motorize my mill, a tight gap would work fine. :rockin:
Why wait till there is a problem to use a blow-off?
I always use a blow-off for the first week of fermentation, no matter the style of beer, nor how much head space the carboy (better bottle, or buckets) has.
But yeah, an airtight seal in the blow off container is a BAD idea. I was happy to...

I recently did a 1.062 IPA that got 76% eff.
Most of my recipes are in the 1.050 range, and their effs are pretty consistent with these results. I haven't brewed anything above 1.070 yet.
I haven't noticed a significant drop in eff with higher gravs. But like...
It never fails, I can plan a recipe for months and hit all my numbers brewing it, but my best beers have always been my "use up the left-over grains/hops/old yeast" batches.
My brew shop sells a large bag that fits my 8 gallon brew kettle with plenty of room to spare. I actually have a lot of extra bag when I use it in my kettle.
